Instructions: Please fill out cover sheet with appropriate information and fax with copy of hazardous manifest to TOTE Hazardous Clerk. Once manifest has been verified, TOTE will fax back cover sheet with notations of approval or disapproval. If approved, load may be released/brought to the TOTE terminal for processing. If disapproved, comments will be made as to what corrections are needed and manifest must be faxed back with appropriate corrections. The shipper must receive an approved Hazardous Authorization Form before the trailer is released/brought to the TOTE facility.
